Annual Enrollment for your 2026 Swift benefits is November 17 ‒ December 1, 2025. Take some time to review your plans to ensure you choose the right coverage for next year. Need help deciding? Use the tools and resources available at swift.benefitsnow.com when enrollment begins November 17.
For 2026, there are several changes, including new medical, life and accident insurance, and disability carriers, effective January 1. Here are the highlights:
- Credence Blue Cross and Blue Shield is replacing Blue Cross Blue Shield of Arizona as the national medical plan carrier. To search if your providers are in-network, visit CredenceBlue.com/Swift, click on Find Care, and browse the directory as a guest under the Blue Card PPO network. Note that CVS Caremark will continue to be the prescription drug carrier.
 - With this carrier change, there will be a few changes to the coverage provided under all three Credence Blue Cross and Blue Shield medical plans:
                        
- Chiropractic care will be limited to 20 visits per calendar year.
 - The Headway network offered through Blue Cross Blue Shield of Arizona will no longer be available. Lucet provides Credence Blue Cross and Blue Shield members with access to in-network behavioral or mental health care providers. For more information, visit lucethealth.com or call 1-877-887-1797.
 - Doctor On Demand, a 24/7 telehealth program, will be available to all Credence Blue Cross and Blue Shield members for a $20 copay/visit for all plans. 98point6 and Hinge Health will no longer be offered.
 - Lantern, our surgery concierge service, will also offer infusion therapy to all Credence Blue Cross and Blue Shield members. Receive care at home or an infusion center, saving money and avoiding long hospital visits. To learn more, call your Care Advocate at 1-855-515-1596.
 
 - Premiums for all Credence Blue Cross and Blue Shield and Kaiser Permanente medical plans are increasing, while premiums for your other benefits are unchanged.
 - Higher Health Care Flexible Spending Account (FSA) limit. For 2026, you can contribute up to $3,400 before-tax dollars to pay for eligible out-of-pocket health care (medical, prescription drug, dental and vision) expenses.
 - Aflac is replacing The Hartford as the carrier for all life and accident insurance, disability, and supplemental benefits.
 - Enhancements are being made to employee and dependent life insurance policies.
                        
- Change in guaranteed issue amount. This is the amount of life insurance for which the carrier does not require evidence of insurability.
                            
- Basic and supplemental life insurance coverage: The $350,000 combined guaranteed issue amount is being unbundled into two separate $300,000 limits.
 - Supplemental spouse life insurance: The guaranteed issue amount is increasing from $10,000 to $25,000.
 
 - Additional coverage options. We’ve included additional coverage options you can purchase for your spouse and child(ren):
                            
- Supplemental spouse life insurance: $50,000
 - Supplemental child life insurance: $5,000 and $10,000
 
 - Basic dependent life insurance. Dependent children, from birth to age 26, and spouses enrolled in a Swift medical plan are provided with company-paid life insurance of $1,000.
 
 - Change in guaranteed issue amount. This is the amount of life insurance for which the carrier does not require evidence of insurability.
                            
 - Annual Enrollment special opportunity! During this year’s Annual Enrollment period only, you can elect supplemental employee and spouse life insurance and disability coverage without having to submit evidence of insurability, up to the guaranteed issue amount,* whether or not you are currently enrolled:
                        
- Supplemental employee life insurance coverage. Up to $300,000
 - Supplemental spouse life insurance coverage. Up to $25,000
 - Short-term disability coverage. Up to any applicable average weekly pay-based limit
 - Long-term disability coverage. Up to any applicable average monthly pay-based limit
 
 
Important action required by YOU during Annual Enrollment:
- You must re-attest your tobacco status each year to receive the credit for the following year.

 - You must confirm if the spousal surcharge applies. If you are covering a spouse on the medical plan and do not confirm, you’ll pay a $1,200 spousal surcharge.
 - Review your beneficiary information for the company-paid life insurance and any supplemental plans you’re enrolled in.
 
Update your info!
Remember: Be sure your mailing address and email address on file with Swift are correct. To update your mailing address, log in to the Swift Transmission Portal and locate the All About Me section. To opt-in to text messaging and update your email, log on to swift.benefitsnow.com. Click the Profile icon (top right) and select Manage Communications to update your mobile number, receive benefits-related text messages and/or change your preferred email address.
Annual Enrollment is also a good time to review and update your life insurance beneficiaries. You can also review your beneficiaries at any time for your 401(k) Retirement Plan at principal.com.

Access your benefits and enroll on the go. With the Alight Mobile app, you can:
- Find important benefits information—Review your current coverage.
 - Choose your benefits—View your options and select the best coverage for you on your desktop, tablet or smartphone.
 - Store your benefit ID cards—Take photos of your benefit ID cards and store them for easy access.
 - Find a doctor or urgent care—Find in-network providers near you based on your plan.

* The regular evidence of insurability rules for these coverages will apply, effective January 1, 2026.
Tobacco-free? That’s worth $1,092.
If you are tobacco-free,* you’ll receive a $21 weekly ($1,092 annual) credit toward your medical plan premiums. Don’t miss out.
You must re-attest your tobacco-free status each year during Annual Enrollment to receive the credit.
*Note: Use of e-cigarettes/vaping is considered tobacco use.
Take part in the Swift Wellness GPS program and save.
Save up to $1,000 on your Swift medical premium by participating in the Swift Wellness GPS program. Spouses enrolled in the medical plan can participate to save up to $1,000 in medical premium credits, too.
Earn your 2026 wellness points September 1, 2025–August 31, 2026. Premium credits will be applied January 1–December 31, 2026. The sooner you start earning, the sooner you’ll see your medical premium credits!
Each point earned = $1. Credits are earned in $200 increments ($200, $400, $600, $800, up to $1,000)and are applied as soon as administratively feasible. View the Swift Wellness GPS Guide.

If you don’t enroll by December 1 and are currently covered through Swift’s medical, dental and/or vision plans:
- You will have the same medical, dental and vision coverage at 2026 rates (and a new carrier if you’re currently enrolled in a Blue Cross Blue Shield of Arizona medical plan option).
 - You won’t participate in the FSAs, either Health Care or Dependent Daycare.
 - You won’t receive our tobacco-free credit of up to $1,092 off the medical plan. Note: Use of e-cigarettes/vaping is considered tobacco usage.
 - If your spouse is currently enrolled in medical, you’ll pay an extra $1,200 for the spousal surcharge.
 
It’s important for you to take action during Annual Enrollment. Review the plans and costs, then select the benefits that are right for you in 2026.
